Forms and Deadlines:
Philanthropy
Registration: coordinators must submit number of service hours and amount
of money donated to the charity within 48 hours of the event’s completion.
Roster
Updates: should be communicated to Program Advisor within one week of the
addition/subtraction.
Forms that are not filled out completely or do not provide enough detail will be returned and must be clarified before the event can be scheduled or approved!
*Remember that these forms are a part of the evaluation process for chapter awards*
