Welcome to the Banner Self Service Information Page!

With your User Name and MyUSI password, you, as a USI employee, will be able to access:

Personal Info: View/update the following: address, marital status, emergency contact, dependents, and campus location information. You also will find information about changing your name.

Benefits: View listing of current benefits.

Payroll: View information on direct deposit, federal tax withholding status, and access form to change your federal withholding. You also will be able to view pay stubs and earnings by salary type.

Employment: View employment history and accrued leave balances.

Reminder: Banner Self Service allows 3 attempts to log on. If you are unsuccessful after 3 tries, you will need to contact the Computer Center at 812/465-1080 or Deb at 812/464-1897 to have your password reset.

To log on to Banner Self Service:

1) Log onto the Internet.

2) Go to www.usi.edu.

3) Click on the MyUSI icon in the lower right hand corner.

4) Enter your User Name and Password. (If you do not know or have forgotten your user name and/or password, contact the Computer Center help desk at 812/465-1080 or Deb at 812/464-1897.) Click OK.

5) Click on the file tab titled Self Service.

6) You should now see one (or more) link(s).

To view vacation/sick/comp balance:

1) Click on Employee on the blue bar.

2) Click on Leave Balances.

To view pay information (for current or past pays after 1/1/05):

1) Click on Employee on the blue bar.

2) Click on Pay Information.

3) Click on Pay Stub.

4) Select the year of the pay you would like to review and click the Display button.

5) Click on the underlined pay stub date to access pay stub information.

To view/change address/phone (including campus building/room and phone number):

1) Click on Personal Information on the blue bar.

2) Click on Update Addresses and Phones.

3) Click on the Current or Primary Address or Phone you would like to update.

4) Change the appropriate information and click the Submit button.

To view/change emergency contact information:

1) Click on Personal Information on the blue bar.

2) Click on Update Emergency Contacts.

3) Click on the individual’s name or add contact by clicking on New Contact.

4) Change or add the appropriate information and click the Submit Changes button.

To view current federal tax withholding information:

1) Click on Employee on the blue bar.

2) Click on Tax Forms.

3) Click on W4 Tax Exemptions or Allowances.

To change current federal tax withholding:

1) Click on Employee on the blue bar.

2) Click on Tax Forms.

3) Click on W4 Tax Exemptions or Allowances.

4) Click on Vendor Web Site. Print form, complete, sign and return to Human Resources.
