SwISTEM Truck Patron Forms

The following forms must be received by the SwISTEM Resource Center from individuals who have attended an orientation session before they will be allowed to utilize SwISTEM equipment and/or resources.

Statement of Assurances Form (to be completed by building administrator)

Teacher Information for Equipment Delivery Form (to be completed by participating teacher)

Participating School Profile Form (to be completed by building administrator)

SwISTEM Equipment Loss/Damage Report (to be completed after a patron loss, damage, or theft of SwISTEM Equipment)
