Case 1

Patient seen in clinic: Mrs. Hertzwen I. Peigh

Exam reveals:

  1. She has an oral temperature of 39oC.
    SIGN   SYMPTOM   NEITHER
  2. BP is within normal limits.
    SIGN   SYMPTOM   NEITHER
  3. Pulse and respirations are within normal limits.
    SIGN   SYMPTOM   NEITHER
  4. She has diarrhea, starting yesterday, about 2-3 x day.
    SIGN   SYMPTOM   NEITHER
  5. She has trouble sleeping.
    SIGN   SYMPTOM   NEITHER
  6. She gets up frequently to urinate, although there isn't much urine, and lately it's a little uncomfortable.. She has not noticed any blood in her urine.
    SIGN   SYMPTOM   NEITHER
  7. A blood count shows that her white blood cell count is a little elevated, and neutrophils are the predominant cell observed. Everything else is normal.
    SIGN   SYMPTOM   NEITHER
  8. Her bones hurt.
    SIGN   SYMPTOM   NEITHER
  9. Her head hurts.
    SIGN   SYMPTOM   NEITHER
  10. She thinks it's the chili con queso she had two days ago to celebrate her one month anniversary that is making her sick.
    SIGN   SYMPTOM   NEITHER
  11. Her last menstrual period was four weeks ago.
    SIGN  SYMPTOM   NEITHER
  12. Abdominal exam is within normal limits.
    SIGN   SYMPTOM   NEITHER
  13. The urinalysis report states that the urine is amber in color, cloudy, positive for white blood cells, protein, sheets of squamous epithelial cells, and bacteria, and within normal limits for everything else.
    SIGN   SYMPTOM   NEITHER
