Apply to USI

In order to apply to USI, you will need:

• A completed Application for Undergraduate Admission.
  
  Apply online
  
  Counselor form - After applying online, print this off and have
      your guidance counselor mail or fax it to the Office of
      Admission.
  - OR -
  Print application - Print this application form and upon
     completion mail to the Admission Office.
  
  
• Official transcripts from all high schools and colleges previously attended.
  
  To expedite the admission process, please ask your guidance counselor to either fax or mail your current transcript to the Office of Admission.
• A non-refundable $25 application fee.
  
  Pay application fee online
  - OR -
  Mail application fee - Print this application fee form and mail
      to the Office of Admission.

To be considered for USI scholarships, submit completed application by March 1.
