Enrollment Procedures

Application for enrollment is made through the Children's Learning Center, where all forms are available. The Children's Learning Center is state-licensed, NAEYC accredited and Level 4 Paths to Quality facility. The center serves children from the ages of two through seven years. Priority enrollments is given to children of USI students, faculty, and staff. Children of alumni and others may be accepted if space is available.

Pre-Admission
Before your child is enrolled, a short pre-admission conference will be scheduled with the Children's Learning Center coordinator or staff member. This meeting will give you and your child the opportunity to get acquainted with the staff and facility, and furthermore gives us the opportunity to get to know you better.

Please note that the center will not admit or maintain any child whose presence can detrimentally affect others or whose needs cannot be met by the Children's Learning Center staff.

Admission
A child will not be admitted to the Center until the following forms are completed and returned:

1. Application Form and $20 Application Fee
2. Intake Agreement Form
3. Free/Reduced Meals Form
4. Emergency Information Card
5. Valid Birth Certificate (USI retains a copy and returns original)
6. Health Record
