| Application for enrollment is made through the Children's Center,
where all forms are available. The Children's Center is a state-licensed
and NAEYC
accredited facility which may serve children ages three through seven
years. Priority enrollment is given children of USI students, faculty, and
staff. Children of alumni and others may be accepted if space is
available.
Pre-Admission
Before your child is enrolled, a short pre-admission
conference will be scheduled with the Children's Center coordinator or
another staff member. This
meeting will give you and your child the opportunity to get acquainted
with the center and staff and will give us the opportunity to get to know
you better.
The center will not admit or maintain any
child whose presence can detrimentally affect others or whose needs cannot
be met by the staff.
Admission
A child will not be admitted to the
Center until the following forms are completed and returned:
Application Form and $20 Application Fee
Intake Agreement Form
Free/Reduced Meals Form
Emergency Information Card
Valid Birth Certificate (USI makes copy and returns original.)
