Diplomas
Formal Application for Graduation
• Each college prepares a Formal Application for Graduation for each potential
graduate, listing graduation requirements (including courses to be completed),
an forwards a copy to the Registrar’s Office. A copy is also sent to the
graduate
• If you have not received a copy of your Formal Application for Graduation,
contact your academic college office immediately.
Diploma Form
• The Diploma Form
requests graduation information
• The Diploma Form is to be completed and returned to the Registrar’s Office in
the Robert D. Orr Center, or you can complete one online.
Both forms must be received by the Registrar’s Office by October 1.
Graduates do not receive diplomas at Commencement. Diplomas may be picked up following Commencement at the Registrar’s Office in the Robert D. Orr Center or student may request that diplomas be mailed. Diplomas (for graduates who complete all requirements and have the Formal Application and Diploma Form on file by October 1) may be picked up at the registrar’s on or after February 12, 2007.
To have diploma mailed, call the Registrar’s Office, 812/464-1761, and inform them of any address changes.
