Disability Verification Forms
In order to qualify as having a disability under federal law, individuals must provide evidence that they have a mental or physical impairment that significantly limits at least one major activity. Student disability documentation must be comprehensive, current and age-appropriate and must adequately verify the nature and extent of the disability in accordance with current professional standards and techniques. Documentation also must clearly identify the student's current functional limitations in order to substantiate the need for specific accommodations. To qualify to use the disability resources in the Counseling Center as well as to obtain our endorsement for in-class accommodations from professors, students are asked to submit one of the following disability verification forms, including either a comprehensive testing report or a recent diagnostic narrative. All forms must be completed by a licensed clinical professional familiar with the history and functional implications of the impairment. Please consult the checklist on our Disability Qualifications page prior to submitting your documentation.
The following forms can be downloaded using Adobe Acrobat and serve as our documentation, since they provide us with the information needed for us to best determine qualification for accommodations. Please note that these forms have requested attachments, without which a student may not be able to complete our registration requirements or qualify for accommodations. Only accommodations that specifically relate to an individual's current functional limitations may be provided.
