Counseling Center office space is available on a limited basis for faculty
members to provide test accommodations to students with disabilities. When
feasible, we also offer the services of the administrative assistant to
proctor exams and assist in locating readers and scribes for those students
who require the accommodation. In order for us to provide this assistance
and ensure students with disabilities receive appropriate accommodations, it
is essential faculty members adhere to Counseling Center procedures:
1. Completion of a Counseling Center Test Instructions form is required for
every test. If there is more than one student taking the same test, only one
form is needed as long as all students’ names are included on the form. You
may download a "double" form by following this link:

2. Please deliver all tests to the Counseling Center the day before testing
unless prior arrangements have been made with the administrative assistant
proctoring the test. Please include two copies of the test if the student
requires a test reader; the extra copy will be returned or destroyed
according to the faculty member’s preference.
3. Receipt or printing of tests sent by electronic mail cannot be guaranteed
due to incompatible software programs, system failures or absent staff, and
this method is not recommended.
4. Please deliver all required testing resources with the test, including
scantrons, formula sheets, maps, slides, diagrams, scratch paper and slide
projectors. Slide projectors should be delivered to the Counseling Center
the day before testing, and are available to order from Instructional
Technology Services (465-7182.)
5. If a test, testing resources or Test Instructions form has not been
received at the time of the scheduled test, the student will be referred
back to the faculty member for other testing arrangements.
6. Retrieval of completed tests is the responsibility of the faculty member.
Choices include: professor pick-up, student return or campus mail. However,
campus mail is not recommended since loss of tests or delivery delays cannot
be controlled.
7. Space is only available for students with documented disabilities who
have completed registration in the Counseling Center.
