Disability Documentation (effective 03-04-10)

Documentation of your disability must be received and your eligibility verified in the Counseling Center before our staff are able to endorse your qualification of academic accommodations with your professors. Please request or download the following Verification of Disability to take to your licensed professional or specialist. This information cover sheet directs your specialist to attach either a Detailed Diagnostic Narrative or a Comprehensive Diagnostic Test Report and send this documentation to the Counseling Center. Once received in our office, your documentation will be evaluated and you will be sent a letter concerning the status of your eligibility.
