Teaching Consultation Request Form

Faculty who are interested in the CAC's Teaching Consultations program want to improve, refine, and/or further develop their teaching. The purpose in providing teaching consulatations is to engage colleagues in a reflective process of formative review. However, we do not conduct summative evaluations (i.e., evaluations of teaching competence for promotion, tenure, or annual reviews).

To help us understand your current needs, please check as many of the consulting activities below that are of interest to you:
