Checklist of Items Needed To Apply:

1. Download and complete the Adult Learner Scholarship application.

2. Attach a brief [300-500 words] essay describing your educational objectives and abilities to combine adult responsibilities [parenting, employment, community engagement] with your academic and career goals. Use standard 12 pt. font size, margins, etc.

3. Provide a letter of recommendation from a member of USI's faculty or staff who supports your application for this award.

4, Complete the electronic FAFSA* and USI's Financial Aid Data Form (this must be completed before February 1, 2010).

5. Submit all materials to:

Please Note:  The deadline for all application materials (Essay and Recommendation) is December 11, 2009 with the exception of FAFSA which is due before 2/1/10.
