Frequently Used Forms

Admission to graduate study requires a completed Graduate Studies application form. Some, but not all, USI graduate programs require additional program-specific application and information forms.

The forms cannot be completed online, but they may be printed directly from the Web page, completed, and submitted to the Office of Graduate Studies by U.S. Mail, along with other application materials.

All applicants are required to submit:

1. a completed application for graduate study
2. a $25 non-refundable application fee made payable to USI, and
3. one official transcript from each undergraduate and graduate institution attended (transcripts will be accepted as official only when sent directly from the registrar of the institution to the Office of Graduate Studies).
