Application Process

Submit an application & be accepted to the University of Southern Indiana.
- You may apply online at http://www.usi.edu/admissn/apply.asp. Your dental hygiene application will not be considered for admission without applying to USI also.

Complete all required prerequisite courses.

Earn a cumulative GPA of 2.8 or higher and a grade of "C" or better in each prerequisite class.

Submit an application to the USI Dental Hygiene Program by February 1st of the calendar year you desire admission to the program.
- Download an application: dentalhygieneapplication.pdf.

Submit official university transcripts from any university other than USI to the dental hygiene program.
- (USI transcripts need not be submitted).

For transfer students:
- As you complete each semester, make sure the dental hygiene program and the University receive updated transcripts.
