2009 Flexible Benefit Plan
Premium Only Plan - Employee contributions for medical insurance may be deducted on a pre-tax basis.
Flexible Spending Accounts - Flexible Spending Accounts allow employees to set aside pre-tax dollars to pay for certain unreimbursed medical and dental expenses, and dependent care expenses.
The University uses The Nyhart Company to administer the flexible benefit plan. Nyhart will provide employees with a secure identification and password to access their flexible spending accounts on-line to check claim status and current balances. Nyhart also will provide participants with a debit card to be used for eligible expenses.
Contributions for calendar year 2009 may be used for purchases through March 15, 2010 for unreimbursed medical, vision, and dental expenses only. The dependent care flexible spending account will still need to be exhausted in calendar year 2010.

Current Participants - All employees who are
currently enrolled in a flexible spending account will need to complete
a new
Benefits
Election Enrollment Form. Please submit the form to Human
Resources by Monday, November 17, 2008.

New Enrollees - Any employee interested in enrolling in a flexible spending account can use a worksheet to help them determine their allocation amount for the uninsured medical and dental expenses or the work-related dependent care expenses. A representative from The Nyhart Company will be at the Open Enrollment and Benefit Awareness Fair, Thursday, November 8, 2007 to assist employees with questions and/or concerns. A Benefits Election Enrollment Form must be submitted to Human Resources by Monday, November 17, 2008.
