Injury and Illness Reporting
If an employee or student worker is injured, an injury and illness form must be completed and returned to the Human Resources Department within 24 hours. If the employee cannot report the injury/illness it is the supervisor's responsibility to do so.
If first aid is required, call campus Security at 464-1845 or if it is an emergency, dial 7777 if on campus. The security officer will complete an injury/illness form.
If first aid is not required, please complete an Injury/Illness form. After completion, please forward this report to Brandy Smith in Human Resources.
Please click here for a list of Worker's Compensation Providers. All injured employees should visit the on-campus Student Health Center, Health Professional Center, HP09. If this facility is unavailable, please use the St. Mary's Convenient Care facility located at 5320 Weston Road, telephone (812) 485-1550. If neither facility is available, the injured employee should go to Deaconess Emergency Room located at 600 Mary Street.
