During Open Enrollment, employees may enroll in health insurance. Current participants may add/delete dependents and/or switch to another insurance plan.
The plans currently offered by the University are the Anthem Blue Access 100 plan, Anthem Blue Access 500 plan, and Welborn HMO.
Each available plan includes dental insurance. The 2006 insurance comparison overview is a good tool for employees to use to compare the health insurance options.
PLAN DESIGN CHANGES FOR 2006:
Anthem Blue Cross/Blue Shield -- Blue Access 100 Plan:
Co-Insurance -- Change from 100% in network to 90%/10% in network, 80%/20% non-network to 70%/30% non-network
Annual out-of-pocket maximum -- Increase from $500/$1,000 to $1,000/$2,000 in-network; $1,000/$2,000 to $2,000/$4,000 non-network
Anthem Blue Cross/Blue Shield -- Blue Access 500 Plan:
No plan design changes for 2006
Welborn HMO
Doctor Office Visits -- Change from 90% after co-payment to 100% after co-payment
Network -- Change from St. Mary's and Deaconess network to Deaconess only

Blue Cross/Blue Shield Blue Access Plan - This plan allows the employee the choice of physicians and facilities/providers both within and outside the network. To view the list of providers in the network, you can use the on-line provider list. Select the Provider Network and then chose the option Anthem Blue Access. There is a $15 doctor office visit co-pay. The monthly premium includes dental insurance coverage with Health Resources, Inc. and is as follows:

Blue Cross/Blue Shield Blue Access 500 Plan - This plan offers employees a lower premium/higher deductible option. Office visit co-payments are $20 and in-network benefits are covered at 90%/10% with a deductible of $500 Single/$1,000 Family. The monthly premium includes dental insurance coverage with Health Resources, Inc. and is as follows:

Welborn HMO - This plan provides service through Welborn Health Plans using Welborn-affiliated doctors. To view the list of doctors, you can search the on-line directory. There is a $15 doctor office visit co-pay for primary care physicians and a $25 office visit co-pay for specialist. The vision insurance is through Vision Service Plan. To select an eye doctor, you can search the VSP on-line directory. You must enter your Social Security Number for Member ID. The monthly premium includes dental insurance coverage with Health Resources, Inc. and is as follows:

Health Resources Inc. - Provides the dental coverage available with all health insurance plans and is included in the monthly cost. The employee can select a dentist who participates in HRI. This plan allows the employee one office visit every six months and anything beyond this is covered at 50% up to $1,200 per person. To see which dentists are in this plan, you can use the HRI on-line network.
