Medical & Dental Insurance
The University offers three insurance plans. Each plan includes dental insurance. The University contributes a portion of the premium and the employee is responsible for the remainder.
The 3 plans currently offered by the University are Anthem Blue Access 200, Anthem Blue Access 500, and Welborn HMO. The 2007 health insurance overview comparison is a good tool for employees to use to compare the four health insurance options.
Anthem -- Blue Access 200 Plan
This plan requires the employee to meet a deductible; $200 per individual/$400 per family for network services and $400 per individual/$800 per family for non-network services. There is a $20 doctor office visit co-pay. This plan allows the employee the choice of physicians and facilities/providers both within and outside the network. Network charges subject to deductible are reimbursed at 90% once the network deductible is met. Non-network charges subject to deductible are reimbursed at 80% once the non-network deductible is met. To view the list of providers in the network, you can use the on-line provider list. Select the Provider Network and then chose the option Anthem Blue Access.

Anthem -- Blue Access 500 Plan
This plan requires the employee to meet a deductible; $500 per individual/$1,000 per family for network services and $1,000 per individual/$2,000 per family for non-network services. There is a $25 doctor office visit co-pay. This plan allows the employee the choice of physicians and facilities/providers both within and outside the network. Network charges subject to deductible are reimbursed at 80% once the network deductible is met. Non-network charges subject to deductible are reimbursed at 60% once the non-network deductible is met. To view the list of providers in the network, you can use the on-line provider list. Select the Provider Network and then chose the option Anthem Blue Access.

Welborn HMO
Welborn HMO provides service through Welborn Health Plans using local affiliated doctors and facilities, and the National Provider Network. To view the list of doctors and facilities, you can search the on-line directory. This plan requires the employee to meet a deductible $100 per individual/$200 per family. Charges subject to deductible are reimbursed at 80% once the deductible is met. There is a $20 doctor office visit co-pay for primary care physicians and a $30 office visit co-pay for specialist. The vision insurance is through Vision Service Plan. To select an eye doctor, you can search the VSP on-line directory. You must enter your Social Security Number for Member ID.

Health Resources Inc.
Provides the dental coverage available with all health insurance plans and is included in the monthly cost. The employee can select a dentist who participates in HRI. This plan allows the employee one office visit every six months and anything beyond this is covered at 50% up to $1,200 per person. To see which dentists are in this plan, you can use the HRI on-line network.
