Fee Waiver Program

Credit Courses
Employees
Eligible employees, employed by the first day of class and enrolled in courses at the University of Southern Indiana may receive: A waiver of full fees for a maximum of 124 attempted credit hours; Full waiver of fees for up to six credit hours for each fall and spring semester and/or four credit hours during each summer session for a total of 15 hours per academic year. Academic year is from Fall through Summer III.
Fee waiver forms are available in Microsoft Outlook (see instructions below). ‘Mac’ computer users should contact Benefits at 464-1781. A Fee waiver form must be submitted for each semester.
Spouse
Spouses of eligible employees pursuing an associate degree, a baccalaureate degree, or a master's degree may receive: A fee waiver of 75 percent of student fees for a maximum of 124 attempted credit hours; Unlimited credit hours per semester

Dependent Children
Dependent Children of eligible employees pursuing an associate degree, a baccalaureate degree, or a master's degree may receive: A fee waiver of 75 percent of student fees for a maximum of 124 attempted credit hours; Unlimited credit hours per semester
A dependent child is defined as a child, stepchild, or a child whose legal guardian is a benefit-eligible employee and on the first day of the academic terms for which the fee waiver is requested: Is 23 years of age or under, Is unmarried, and Meets the IRS Support test as a dependent of the employee or spouse
Proof of guardianship may be required. Proof of dependency is required.
ELECTRONIC FEE WAIVER FORM/INSTRUCTIONS
To complete the electronic fee waiver form, open the employee Outlook e-mail
system as follows:
Tools - Forms --> Choose a Form At the top of the screen in the ‘Look In’ drop
down list, choose Organizational Forms Library. Select: HR Class Fee Waiver –
Dependent; or HR Class Fee Waiver – Employee form. Open selected form. Complete
form on-line.

Non-Credit Courses
Non-credit courses are provided through Continuing Education in Extended
Services. Eligible employees may receive a full fee waiver for fitness/wellness
and other non-credit continuing education courses which include most leisure
activity courses (e.g., foreign language, crafts, water aerobics, etc.). When
non-credit courses are offered through third-party providers, the university
will ordinarily waive only its portion of the fee. An application form for
non-credit fee waiver may be obtained from Human Resources. All forms must be
returned to Amy Miller in Human Resources-Benefits.
