Optional Vision Coverage is available to employees through UnitedHealthCare Vision which is a network provider plan and does have cost limitations for eye exams, lenses, and frames. $10 Co-payment for an annual eye exam
Contact or eye glass lenses available every 12 months, subject to maximum limits
The monthly premium for the optional vision coverage with Spectera is as follows:
