• Pay the $100 SEVIS fee if applicable.

• Obtain J-1 visa at American Embassy or consulate with DS-2019 Form issued by USI or program sponsor.

• Return USI Resident Life/Food Service Contract (with $200 deposit) to reserve a room on campus.

• Notify International Programs and Services or ESLI of your arrival plans.

• Complete the Student Immunization Form with a doctor's signature.  You can send this form to the International Programs and Services office prior to arrival, or you can bring it with you.

• Purchase health insurance from USI upon arrival or provide a certificate of insurance (in English) as proof of alternate insurance coverage.
