Online Application

This form works best with Internet Explorer

Due November 15, 2012

All history majors/minors and social science teaching majors are invited to apply.  This form helps the history department faculty to select recipients of departmental awards and to nominate students for Liberal Arts or university-wide awards on Honors Day.

Note: Some university-wide awards, such as the Pamela Grubb Memorial or the Kappa Delta Pi Award, have their own application form.  For more information on University-wide awards, please visit the Scholarships and Awards website at http://www.usi.edu/libarts/scholarships.asp.

(If you prefer to fill out a hard copy of the application, you may download it.  The application will need to be submitted to any History advisor, the Department Administrative Assistant, or the Department Chair before the deadline.)
