Case Management Conference at USI May 12

Brandi Schwartz Marketing Coordinator Outreach & Engagement 812/464-1854 Cheri Lattimer, registered nurse and executive director of the Case Management Society of America, will deliver the keynote address, "The Role of the Case Manager in Transitions of Care and Reducing Avoidable Readmissions" at 8:15 a.m. Lattimere will discuss how case manager interventions improve the communication and process of transitions to reduce preventable hospital readmissions. At 10 a.m., Dr. Malaz Boustani, a founding physician of the Healthy Aging Brain Center at the Center for Senior Health at Wishard Health Services in Indianapolis, will speak about the Healthy Aging Brain Center and how health system costs can be reduced through a memory care practice. Registered Nurse Faith Roberts will present, "Closing Plenary: It's in Every One of Us" at 2:45 p.m. which will discuss today's stressful lifestyle and the positive impact of a workplace hero. Other sessions topics will include telehealth, guardianship and estate planning, multiple sclerosis, documentation for case managers, epicondylitis, and improving patient adherence through enhanced interviewing skills. Conference registration is $95 and includes materials, lunch, and refreshments.
