First Name, Middle Initial	,
University ID No. ((Must include leading zeros. ex. 000001234))
	Faculty/Staff	Student
Vehicle Color
Vehicle Year
Vehicle Make (DO not include model. Ex. Dodge, not Dodge Neon.)
Vehicle Type	Car Truck Motorcycle
License Plate Number (Do not include dashes, symbols or spaces)
License Plate Expiration Date (Use format YYYY ex. 2005)
License Plate State

Your vehicle will now be registered with USI's Parking Department.
