Diploma Forms
Meeting graduation requirements and formally applying for graduation is each student's responsibility.
Candidates for graduation should file with the Office of the Registrar both a Formal Application for Graduation (obtained at the office of the Dean of the candidate's major) and a Diploma Form. If degree requirements will be completed in a spring or summer term, the forms should be submitted no later than the third week of the preceding fall term. If degree requirements will be completed in the fall term, the forms should be submitted no later than the third week of the preceding spring term. Timely submission of the Formal Application for Graduation and Diploma Form help ensure that degree candidates are included in commencement mailings and the commencement program, will be considered for commencement honors, will have a diploma ordered, etc.
The Diploma Form contains your graduation information, including your name as you want it to appear on your diploma and in the commencement program (proper spelling, upper/lower case, middle name or initial, etc). If no Diploma Form is submitted, the student's current name on record with the University will be used when printing the commencement program and diploma. Students must submit the Diploma Form online - requires electronic signature (myUSI password)
Diploma Forms submitted late (after the following dates) may cause a delay in the availability of your diploma:
Fall 2009 graduates - October 1, 2009
Spring or Summer 2010 graduates - March 1, 2010
Students may pick up their diploma at the Office of the Registrar in the Orr Center, or request that their diploma be mailed.
