Diploma Forms

Meeting graduation requirements and formally applying for graduation is each student's responsibility.

Candidates for graduation should file with the Office of the Registrar both a Formal Application for Graduation (obtained at the office of the Dean of the candidate's major) and a Diploma Form. If degree requirements will be completed in a spring or summer term, the forms should be submitted no later than October 1 of the preceding fall term. If degree requirements will be completed in the fall term, the forms should be submitted no later than March 1 of the preceding spring term.

Timely submission of the Formal Application for Graduation and Diploma Form help ensure that degree candidates are included in commencement mailings and the commencement program, will be considered for commencement honors, will have a diploma ordered, etc.

The Diploma Form contains your graduation information, including your name as you want it to appear on your diploma and in the commencement program (proper spelling, upper/lower case, middle name or initial, etc). If no Diploma Form is submitted, the student's current name on record with the University will be used when printing the commencement program and diploma. Students must submit the Diploma Form online - requires electronic signature (myUSI password)

Ready to graduate in Fall 2013?

1) Submit a Formal Application for Graduation form by March 1, 2013. If you
have not yet initiated this process, contact your academic advisor immediately.

2) Complete a Diploma Form online by March 1, 2013.

3) If you wish to have your diploma mailed when it becomes available, create a
Diploma Mailing Address through myUSI (my.usi.edu).

NOTE: Diploma Forms submitted late may cause a delay in the availability of your diploma.

Students may pick up their diploma at the Office of the Registrar in the Orr Center, or request that their diploma be mailed.
