Withdrawal from the semester/term for medical reasons
A student seeking to withdraw from the semester/term, because of medical reasons, must complete the procedure outlined below:
At the Registrar's Office, students may obtain the Application for Medical Withdrawal form, which must be completed and signed by the treating medical provider. A student may seek to withdraw from the entire set of courses for the term, or may request selective withdrawal from specific courses. In the case of selective withdrawal, the treating medical professional must articulate why the medical condition affects performance only in certain courses, thus warranting a selective withdrawal. By seeking withdrawal because of medical reasons, the student consents to the release of individually identifiable health information and other medical records and information governed by the Health Insurance Portability and Accountability Act of 1996 ("HIPAA") to the Registrar, faculty, Administrative Appeals committee and other University personnel for the purposes of reviewing the student's request for withdrawal.
Once the completed form and doctor's statement has been received by the Registrar, the faculty members for each course for which a withdrawal is requested will be contacted. Each faculty member will be allowed to review materials related to the request, and will be given 10 business days to respond to the Registrar in support or opposition of the withdrawal request. After the 10-day limit, if no faculty member has concerns or objections the withdrawal shall be granted. If a faculty member opposes the withdrawal, then the request, along with the faculty member's concerns, will be forwarded to the Administrative Appeals Committee for final disposition. If, for some reason, the faculty member is unavailable to respond to a request, then the chair of that academic department will be asked to respond on behalf of the faculty.
Requests for a Medical Withdrawal for a term/semester prior to the current term will be forwarded to the Administrative Appeals Committee for review and final decision.
NOTE: No refund of tuition and fees (full or partial) will be given more than one calendar year after the end of the academic year for which the medical withdrawal is requested. For example:
