USI Transcripts
Academic transcripts are maintained by the Office of the Registrar. Official transcripts include student name, USI student ID number and social security number, date of birth, permanent address, all officially enrolled and dropped courses, grades received, academic standing, majors, minors, degrees awarded, graduation honors earned, attainment of honors list, transfer institutions, number of transfer hours accepted, cumulative and semester statistics, quality hours, quality points, earned hours, and GPA.
There is no charge for a transcript, but requests are limited to five copies per day (please request only as many as are actually needed).
A transcript will not be released if a student has an unmet financial obligation to the University. A transcript will also be withheld for Stafford Loan recipients who have failed to complete an exit counseling session with the Office of Student Financial Assistance.
Transcript requests are processed in the order they are received. Transcripts to be mailed may take up to 2 business days to process (or longer during high volume time periods). Transcripts requested to be picked up will be available the next business day following receipt of the request.
Requesting that a transcript be sent to another institution does not automatically indicate that the student wishes to be withdrawn from current or pre-registered courses. Any student wishing to withdraw must officially initiate withdrawal procedures.
The University can release only official USI transcripts. Transcripts from other universities or high schools attended must be requested directly from those institutions.
Students can view their semester grades and unofficial transcript online via myUSI. If an official transcript is needed, students should follow the request instructions below.
Requesting your official transcript
The Family Educational Rights and Privacy Act of 1974 requires the written signature or electronic signature (myUSI password) of the student to release information pertaining to their academic records of the student. Submission of a transcript request by any of the following means conveys consent to the officials of the University of Southern Indiana to release an official transcript of your permanent academic record.
Students can request their official USI transcript via any of these options:
Submit a request online via myUSI (my.usi.edu) Students who have been enrolled during the most recent tax year should submit a transcript request electronically. Log into myUSI with your username and password, and:
Click the Self Service icon on the dashboard
Click Student, then Student Records
Click Request Printed Transcript to submit a request
Carefully follow each page's on-screen instructions to walk through the request process
View Status of Transcript Requests allows you to check the details of any requests (date printed, number of copies, recipient, etc.)

Mail a request letter to the USI Office of the Registrar including the following information:
Full Legal Name (including all names attended under)
Social Security Number (required for processing)
A telephone number, e-mail address, or mailing address where we may correspond in case of questions/problems with request
Number of copies requested
Address to which the transcript should be mailed (transcripts are not faxed)
Student's full legal signature
