 Know the Code and Know YOUR Rights

Refer to the Students Rights and Responsibilities Handbook, A Code of Student Conduct for detailed descriptions of the rules and regulations for the University of Southern Indiana and Department of Housing and Residence Life.

 There are a number of forms and information sheets you will encounter as you move through the university conduct process. Below you will find an explanation as to how forms are used.  In many cases an example form is available.

Notice of Infraction (PDF)
A notice of infraction is a formal, written statement that a student’s actions are not acceptable and that subsequent infractions will result in more severe action.  This form is delivered with a copy of the incident report.  The notice of infraction lists the policy or policies violated and lists the sanction that is assessed to the resident.  If the resident does not agree with the finding, he or she can request a full administrative meeting with the administrator who sent the notice.  

Meeting Notice (PDF)
A notice of administrative hearing is a formal, written request for a meeting with an administrator in Residence Life to discuss the alleged incident that was documented.  This notice is accompanied by a copy of the incident report and important information that outlines the rights of the resident during the process.

Meeting Record (PDF)
This is the form used by the administrator to note any sanctions that resulted from the alleged violations.  If there are no sanctions it is noted on the form.  It also contains valuable information on the appeals process.

Consent to Release Form (PDF)
This form explains the policies and procedures with respect to the Family Educational Rights and Privacy Act of 1974, U.S.C. 1232.  It is allows students to express their written consent to allow specific persons or parties access to their confidential information.
