We strongly encourage you to complete the on-line contract. However, if you would like a contract mailed to you, please request a paper contact by emailing living@usi.edu or by printing off the above .pdf (Adobe Acrobat) version of the housing contract.

Change your apartment/res hall room or request a roommate - Apartment Change/Roommate Request Form - For Spring 2009 Semester ONLY (not to be used for Fall 2009 Contract Renewal changes).
Holiday Break request - to stay in Housing over the Winter Break - Will go active in November 2009
Request to enter housing before the designated period: Early Check-In begins August 24. Pre-Approval from Housing and Residence Life is required.
Request to stay in housing beyond the end of the Semester - will go active in November 2009

Housing Contract Cancellation Forms:

Cancel contract and/or Deposit Request Form
Cancellations are being accepted without penalty until April 1st, 2009 (after April 1st, 2009 the $200 housing deposit is forfeited)

For information on meal plans please contact the food service office at (812) 464-1859 All meal plan adjustments or changes are handled by the food service office.
