Paperwork to Start a New Club

When you are ready to move forward with this process, use the following list to help you stay on task.

After you sign up for an account on Orgsync, you will need to meet with Kathy Jones in Student Development.

	Intent to organize (Orgsync) (links to Student Development)
	Officers / CPR & First Aid certified list
	RFW Waivers/member info (print & fill out completely for each member)
	Coach(es)/Advisor code of conduct form signed
	Finances (Open an agency fund)
	Funding Sources (check with the RFW and the SGA)
	Constitution and Bylaws (create your own) (example)
	OPTIONAL: Medical Insurance card for each member (photocopy)
	OPTIONAL: Identification for each member (State ID photocopied)
	Grades /  Non Hazing Release form printed and signed by all members
