Insurance A Certificate of Insurance shall be provided to the Office of Scheduling
Services, naming the University of Southern Indiana as additional insured with
an original signature of an authorized insurance company representative and
shall provide for at least 10 days notice of change in coverage or
cancellation. The University must receive the Certificate of Insurance at
least 30 days prior to the start of the event.
Persons using University space agree to comply with all applicable
requirements of the American Disabilities Act (ADA) in assuring the
availability of auxiliary aids and services required by any employees and
attendees of the event. Persons shall be solely responsible for the cost of
any such auxiliary aids and services, and they will be required to hold
harmless the University of Southern Indiana, its officers, directors,
employees, and agents from and against any claims resulting from the failure
to comply with ADA standards for access to its programs and services.

insurance in full force and effect during the requested date(s) of an event
with an insurance carrier licensed to do business in the State of Indiana. The
policy should provide general liability coverage in the amount of $300,000 and
coverage for all participants.
