-
Attendance will be taken at every class meeting; it is imperative that you are present and prepared for every meeting of this class.
- Your absences will be monitored and when the number of your absences from this class exceeds the number of times the class meets per week, procedures will begin to administratively withdraw you from class. (The maximum number of absences allowed in a summer term is 2.)
- Excessive tardiness will be treated as an absence.
- Your class attendance will be validated by your signature in the attendance notebook; it is your responsibility to sign this roster at the beginning of class. (If you sign the roster and then leave the class, the instructor will make note of this and you will be counted absent.)
If you miss class you should obtain documentation for that absence and keep it in case it is needed for the appeal process. All such documentation will be submitted by the student to the Academic Coordinator located in the Department of Mathematics at the time of the appeal.
The instructor monitors the number of absences, and once you
have exceeded the maximum number of absences permitted, will
initiate the withdrawal procedures. All absences from class are
included in this total reported to the Academic Coordinator.
This includes absences
due to university-sponsored activities, such as participation in
athletic events, field trips, etc. The documentation for each
absence will be reviewed during the appeal process and a
determination made at that time. It is your responsibility to plan
ahead if you know that you will need to miss class for an event that
may not be excused or for which you will be unable to obtain
documentation.
Steps in the administrative withdrawal/appeal process:
- Using USI email accounts you will be notified that you are being withdrawn from the class; the
Academic Coordinator will also be notified.
- You will have 24 hours* from the time that the email was sent, not counting weekends or holidays, to notify the
Academic Coordinator that you plan to appeal the administrative withdrawal. Failure to read your e-mail is not an acceptable excuse for missing this deadline. You should continue attending class during the appeal process.
- Once the Academic Coordinator has been notified that an appeal will be made, documentation should be submitted as soon as possible, but no later than one week after your notification.
Only written documentation will be accepted. For example, if you were ill, you need a doctor’s note; if you had jury duty, you need verification from the courts; if you attended a funeral of a relative, you will need the obituary notice; if you had an automobile accident; you will need a copy of the accident report.
- If your appeal is approved, you will continue attending the class and the process for administratively withdrawing you from the course will end.
- However, if the appeal is denied by the Chair of the
Department of Mathematics, you will have 24* hours from the time that you were notified to inform the
Associate Dean of the Pott College of Science and Engineering that you are appealing your withdrawal from the class. You then have two days, not counting weekends and holidays, to provide written documentation to the
Associate Dean.
- If your appeal to the Associate Dean is approved, you will continue attending class; if your appeal is denied, you will be withdrawn from class.
- If the administrative withdrawal is not appealed or if the appeal is denied, the registrar’s office will be notified and you will no longer be permitted to attend the class. If you are due a refund (that is the withdrawal is during the first four weeks of class) it will be calculated using the date that the withdrawal process was initiated. If you are withdrawn prior to Friday of the ninth week, your grade will be a W; if you are administratively withdrawn after this date, your grade will be a W only if you are passing the class; otherwise you will receive a failing grade.
* This time frame will be honored unless it is the last week of class. If the appeal process falls at the end of the semester, then all paperwork must be submitted no later than 4:30 p.m. on Thursday prior to the week of final exams.
