Faculty Research & Creative Work Awards (FRCWA)

The FRCWA final report is due 30 days after the project end date (either February 14th or July 14th). Submit one original and one electronic copy to the Sponsored Research Office. The final report should be no more than six pages; use a type font no smaller than 10 pt; and include the following information:

A. Title page with award information (1 page):

Project Account Number,
Project Director's (PD's) Name(s),
Project Title,
Start Date,
Original End Date,
Award Amount,
Final End Date if extended, and
Final Report Date

B. Description of project results (3-page maximum):

Account for differences, if any, between proposed and actual project, addressing items in the proposal guidelines.
Address how successfully each objective was met.
Describe any differences in your goals or plans as a result of the project; what difference(s) has this funding made.

C. Financial report (questions 2-6 -- 1 page maximum):

Include an itemized budget of expenditures (explain how the funds were actually spent, items purchased, etc).
How well did the funding work?
Was the amount sufficient?
What implications did the funding have for your project?
Did the award amount exceed your needs? Why?
With hindsight, would you change the budget request? If so, how?

D. List any publications, reports, or presentations generated by the project. (1 page maximum)

Extension Request Guidelines

If the project director (PD) will be unable to complete the project before the project end date and unencumbered funds remain, the PD can either (a) return the funds or (b) request an extension. The PD should arrange for disposition of the funds before the project end date.

To return the funds, simply notify the Grant Accountant (GA) that the remaining funds will not be required. The GA will return the unspent funds to the FRCWA balance. See the Staff & Contacts page for the name and phone number of the current Grant Accountant
To request an extension, the PD should send an email to the Sponsored Research Office. SRO will notify the FASTRC Chair and the GA. (If the request is received during the summer, the request will be held until new FASTRC resumes work in the fall.) The FASTRC Chair will obtain FASTRC decision on the request and notify the PD, SRO, GA, and the VPAA's office.

The extension request should give the following information:

PD Name,
project title,
original start & end dates,
original award amount,
account number,
a short paragraph explaining the reason for the extension request,
a brief update of your progress on the project, and
a proposed new end date (must be either January 15th or June 15th).
