TRYOUTS CHECKLIST

check Read through and sign the USI Cheer Team 2008 Application Packet prior to tryouts.

checkEvery applicant must have a basic medical physical - performed at his/her own expense (no blood work or urinalysis required). For those individuals who were part of the Cheer Team during the previous season still must complete another physical and bring the paperwork with them the day of tryouts.

check Every applicant must present proof of medical insurance. You must have insurance cards or information on hand to fill out additional paperwork.
