Office Visit Plan (OVP)
To meet the needs of students both with and without private health insurance, we have developed an office visit plan (OVP) for a fee of $40 per semester. The OVP covers all office visits incurred during a semester. The plan was developed for the convenience of our student who need easy access to a medical provider between classes, or while away from home. It eliminates any co-payment that may be necessary with other insurance providers and provides discounted fees on certain procedures. Enrollment forms for the OVP must be completed and the $40 fee paid to be effective for the semester.

There are two ways to enroll in the OVP:

  1. All students living in University housing are automatically enrolled in the OVP. The $40 OVP fee is automatically charged to the student’s account for every semester that he is consecutively enrolled at USI. Students who move out of University housing may request to drop the OVP. See the paragraph below regarding deadline dates.
  2. Students living off campus may print out this form and submit it to the Student Health Center, Health Professions Building room 091, 8600 University Boulevard, Evansville, IN 47712. The $40 OVP fee will then be automatically charged to the student’s account for every semester that he is consecutively enrolled at USI.

To be eligible for a credit, cancellation, or refund of the current semester’s health services fee, drop requests must be received in the Student Health Center by the dates listed below. The student must not have used the services of the Student Health Center within the first four weeks of the current semester. Drop requests received after these dates will take effect the following semester. Student may not drop the plan as long as they live in University housing.

Friday, September 21, 2007, for fall semester 2007
Friday, February 8, 2008, for spring semester 2008
Thursday, May 22, 2008, for summer I 2008
Thursday, June 26, 2008, for summer II 2008
Thursday, July 31, 2008, for summer III 2008

Students who have withdrawn from the OVP may request to be reinstated by contacting the Student Health Center office at 812/465-1250.

To withdraw from the Office Visit Plan, you may print and complete this drop request form then return to our office.

Services Covered by the OVP

  1. Unlimited visits with the medical staff for illness or injury.
  2. Physical examinations needed to complete required forms for athletic participation and health professions majors and/or outside agencies. There may be additional cost depending on services required for completion of these forms.
  3. Allergy injections can be administered by the USI Student Health Center's nursing staff. Hours for injections are 8 to 11:30 a.m. and 1 to 4 p.m., Monday through Friday. Summer hours are 9 a.m. to 3 p.m.  Allergy serum must be brought to the center, along with written instructions from the allergist. Instructions must include the dosage schedule and an identifying label that includes the patient's name and vial number, and the physician's name and address. Any changes in the dosage schedule require written authorization from the allergist.
