Medical Records
The first time you visit the USI Student Health Center you will be asked to complete a health history form. This begins your medical record which becomes a history of your visits to the health center. Each time you come to the health center, your symptoms, the clinician's observations, the diagnosis, and the treatment plan will be recorded in your medical record. Telephone conversations and electronic mail communication related to your healthcare also will be documented.
Your USI Student Health Center medical records are strictly confidential!
Records will not be released to anyone without the student's signed authorization. If a student wants us to discuss healthcare issues with his/her parents, a release form must be signed by the student.
