
Transcripts
Academic transcripts are maintained by the Office of the Registrar. Official transcripts include student name, social security number, date of birth, permanent address, secondary school attended all officially enrolled and dropped courses, grades received, academic standing, majors, minors, degrees awarded, graduation honors earned, attainment of honors list, transfer institutions, number of transfer hours accepted, cumulative and semester statistics, attempted hours, quality hours, quality points, earned hours, and GPA.
Requesting your Transcript
The Family Educational Rights and Privacy Act of 1974 requires the written
signature or electronic signature (USI Security Code Number-PIN) of the
student to release information pertaining to their academic records of
the student. Submission of a transcript request by any of the following
means conveys consent to the officials of the University of Southern Indiana
to release an official transcript of your permanent academic record.
You can request your USI transcript via the following options:
1. Submit an on-line Transcript Request form.
2. Print a Transcript Request Form and fax to (812) 464-1911 or mail to USI Office of the Registrar. You will need Adobe Acrobat Reader to read and print this form. If you do not have it, please select the Adobe Acrobat Reader Icon below to download the software.
3. Visit the USI Office of the Registrar and fill out a Transcript Request Form.
4. Mail a letter to the USI Office of the Registrar including the following information:
* Full Legal Name (including all names attended under).
* Social Security Number.
* Address to which the transcript should be mailed.
* A telephone number, or e-mail address, or mailing address where we may correspond about this request.
* Full legal signature
* Number of CopiesMail to:

Note: There is no charge for a transcript. A transcript will not be released if a student has an unmet financial obligation to the university. A transcript will also be withheld for Stafford Loan recipients who have failed to complete an exit interview with the Office of Student Financial Assistance.
Transcripts are processed in the order they are received. Transcripts to be mailed may take 2 business days to process. Transcripts requested to be picked up will be available the next business day following receipt of the request.
The University can release only official USI transcripts. Transcripts from other universities or high schools attended must be requested directly from those institutions.
