HR outlines process for alternative work arrangements or considerations
As a result of the COVID-19 pandemic, most employees have been away from the USI campus since March 17. We are approaching the time for a safe return to campus beginning July 6 (based on guidance available today). The Human Resources (HR) process for submitting a request for alternative work arrangements or considerations is now available for the period between July 6 and the end of the Fall semester, December 22.
The University is committed to ensuring that your submission remains private. This option is strictly voluntary. The information you provide will be reviewed by HR and only approved HR employees will have access to your medical information. You will be contacted by HR or your supervisor regarding the alternative work arrangements or considerations you are requesting.
Who is eligible?
Older employees [defined as age 65 and older by the Centers for Disease Control and Prevention (CDC)] and employees of any age who have serious underlying medical conditions might be at higher risk for severe illness from COVID-19, and may contact HR to request alternative work arrangements or considerations. HR may also be contacted to submit a request for alternative work arrangements or considerations, if an employee has a household family member in any of the high-risk categories outlined by the CDC.
• 65 years of age or older
• Chronic lung disease or moderate to severe asthma
• Serious heart condition
• Severe obesity or a body mass index (BMI) of 40 or higher
• Chronic kidney disease and undergoing dialysis
• Liver disease
• Condition which causes me or a household family member to be immunocompromised
What do I do if either I or a household family member qualify?
You may choose to complete the form that was emailed from HR. Your information will be submitted directly to HR. Again, submissions are entirely voluntary; for example, even though you are 65 years of age or over, you may choose not to submit the form to request alternative work arrangements or considerations.
In order to process requests as soon as possible and before return to campus, those who wish to submit the form are asked to do so no later than Thursday, June 11 by 4:30 p.m. We recognize this is only a few days, however it is necessary to adjust the Fall 2020 class schedule. Please submit your information to HR as soon as possible.
If you have any questions about the form or the process, please contact Lynn Melms at email@example.com or Doug Goeppner at firstname.lastname@example.org. Again, for those who qualify and wish to submit, we ask you to do so by Thursday, June 11.
To complete the form, find the link that was emailed to you from Human Resources. If you have trouble locating the form, contact Human Resources.
