USI employees, students and guests/visitors to campus are asked to self-monitor and check daily for symptoms of COVID-19. Anyone with symptoms of COVID-19, who has been exposed to someone with COVID-19 or has side effects 48 hours after a vaccination, should fill out the self-reporting form, remain off campus and contact their primary care physician for evaluation. Expect a follow-up call from the Dean of Student Office (for students and student workers) or Human Resources (employees) after completing the self-report form.

You may also voluntarily submit your vaccination record at any time.

Student (and Student Worker) Contact Tracing

Vaccinated

Unless otherwise directed by a contact tracer.

Positive for COVID-19

Student isolates for 10 days. There is no early out option.

Displaying COVID-19 symptoms (but is not getting tested)

Student ISOLATES for 10 days. The only early out option is with an alternate diagnosis.

Symptoms (awaiting test results)

Student ISOLATES for 10 days.

If test is negative - student is clear to return to classes.

If test is positive - student isolates for 10 days from the onset of symptoms.

Asymptomatic

Exposed to a positive case or identified as a close contact. No quarantine required. Eligible to take COVID-19 test days 5-7 (PCR or Antigen).

Negative - Clear to return to classes.

Positive - Isolate for 10 days from test result.

Not Vaccinated

Unless otherwise directed by a contact tracer.

Positive for COVID-19

Student isolates for 10 days. There is no early out option.

Displaying COVID-19 symptoms (but is not getting tested)

Symptoms (awaiting test results)

Student ISOLATES for 10 days.

If test is negative - student remains in isolation. The only early out option is with an alternate diagnosis.

If test is positive - student isolates for 10 days from the onset of symptoms.

Asymptomatic

Exposed to a positive case or identified as a close contact. Quarantine for 10 days. Eligible to take COVID-19 test days 5-7 (PCR or Antigen).

Negative - Out on day 8.

Positive - Isolate for 10 days from test result.

Employee 

Exposed is defined having been in close or direct contact (within 6 feet), with or without wearing a face covering, for a total of 15 minutes or longer cumulatively within a 24-hour period with someone who has a confirmed COVID-19 positive test result.

Fully Vaccinated

Unless otherwise directed by a contact tracer.

Exposed, but not experiencing symptoms.

Action: Vaccination card on file, no quarantine (not required to leave or remain off campus).

CDC recommends testing 5-7 days after exposure. If positive result, employee completes new Employee Self-Report for ‘Positive Case’.

If symptoms develop, employee completes new Employee Self-Report for ‘Symptomatic regardless of vaccination status’.

Experiencing COVID-19 symptoms regardless of vaccination status.

Action: Remain off campus for 10 days from COVID-19 symptom(s) onset and 24 hours with no fever without the use of fever-reducing medication and other symptoms of COVID-19 are improving. May be released to return to work/campus sooner if you provide us alternative diagnosis documentation from your healthcare provider stating your symptoms are unrelated to COVID-19.

I have a confirmed case of COVID-19.

Action: Remain off campus for 10 days from positive test date.

Not Fully Vaccinated

Unless otherwise directed by a contact tracer.

Exposed, but not experiencing symptoms.

Action: Remain off campus, may test on days 5-7, if negative result, return on day 8. If no test, return after day 10.

Experiencing COVID-19 symptoms regardless of vaccination status.

Action: Remain off campus for 10 days from COVID-19 symptom(s) onset and 24 hours with no fever without the use of fever-reducing medication and other symptoms of COVID-19 are improving. May be released to return to work/campus sooner if you provide us alternative diagnosis documentation from your healthcare provider stating your symptoms are unrelated to COVID-19.

I have a confirmed case of COVID-19.

Action: Remain off campus for 10 days from positive test date.
