USI employees, students and guests/visitors to campus are asked to self-monitor and check daily for symptoms of COVID-19. Anyone with symptoms of COVID-19, who has been exposed to someone with COVID-19 or has side effects 48 hours after a vaccination, should fill out the self-reporting form, remain off campus and contact their primary care physician for evaluation. Expect a follow-up call from the Dean of Student Office (for students and student workers) or Human Resources (employees) after completing the self-report form.

You may also voluntarily submit your vaccination record at any time.

Student (and Student Worker) Contact Tracing

Vaccinated with Booster or primary shots not eligible for booster yet (6 months – Pfizer or Moderna; 2 months J&J)

Unless otherwise directed by a contact tracer.

Positive for COVID-19

Student ISOLATES for 10 days. If after staying isolated for 5 days, your symptoms have resolved or they are significantly resolved, you can contact the Dean of Students office using this form and they will be in touch to evaluate your potential return to classes/campus. If you return to campus you will need to continue to wear a mask at all times when in public places or around others for an additional 5 days.

Displaying COVID-19 symptoms (but is not getting tested)

Student ISOLATES for 10 days. If after staying isolated for 5 days, your symptoms have resolved or they are significantly resolved, you can contact the Dean of Students office using this form and they will be in touch to evaluate your potential return to classes/campus. If you return to campus you will need to continue to wear a mask at all times when in public places or around others for an additional 5 days.

Symptoms (awaiting test results)

Student ISOLATES for 10 days.

If test is negative after five days from onset of symptoms, student can request to be evaluated to return to classes/campus using this formIf you return to campus you will need to wear a mask for an additional 5 days.

If test is positive - student isolates for 10 days from the onset of symptoms. If after staying isolated for 5 days, your symptoms have resolved or they are significantly resolved, you can contact the Dean of Students office using this form and they will be in touch to evaluate your potential return to classes/campus. If you return to campus you will need to continue to wear a mask at all times when in public places or around others for an additional 5 days.

Asymptomatic

Exposed to a positive case or identified as a close contact. No quarantine required. Should test on day 5 if possible.

Positive - Isolate for 10 days from test result.

Not Vaccinated, Vaccinated Without Booster

Unless otherwise directed by a contact tracer.

Asymptomatic

Exposed to a positive case or identified as a close contact. Quarantine for 10 days. Should test on day 5 if possible.

If after staying in quarantine for 5 days, you have no symptoms, you can contact the Dean of Students office using this form and they will be in touch to evaluate your potential return to classes/campus. If you return to campus you will need to continue to wear a mask at all times when in public places or around others for an additional 5 days.

Positive - Isolate for 10 days from test result.

Employee

Exposed is defined having been in close or direct contact (within 6 feet), with or without wearing a face covering, for a total of 15 minutes or longer cumulatively within a 24-hour period with someone who has a confirmed COVID-19 positive test result.

Up-to-Date on COVID-19 vaccinations

  • Fully vaccinated (primary shots) with booster OR
  • Ineligible for booster until 5 months-Pfizer or Moderna; 2 months-J&J after fully vaccinated.

Unless otherwise directed by a contact tracer.

Exposed, but not experiencing symptoms.

Action: Vaccination card on file, no quarantine (not required to leave or remain off campus).

CDC recommends testing on day 5 after exposure. If positive result, employee completes new Employee Self-Report for ‘Positive Case’.

If symptoms develop, employee completes new Employee Self-Report for ‘Symptomatic regardless of vaccination status’.

Experiencing COVID-19 symptoms regardless of vaccination status.

Action: Remain off campus for at least 5 days from COVID-19 symptom(s) onset and 24 hours with no fever without the use of fever-reducing medication and other symptoms of COVID-19 are improving. May be released to return to work/campus sooner if you provide us alternative diagnosis documentation from your healthcare provider stating your symptoms are unrelated to COVID-19.

I have a confirmed case of COVID-19.

Action: Remain off campus for at least 5 days from positive test date.

Not Up-to-Date on COVID-19 vaccinations

  • Unvaccinated (primary shots) OR
  • Fully vaccinated (primary shots) without booster.

Unless otherwise directed by a contact tracer.

Exposed, but not experiencing symptoms.

Action: Remain off campus for at least 5 days from exposure day.
