Advance Care Planning Conference

Due to COVID-19 restrictions we will not host a live event but plan to offer virtual options with advance care planning content.  Information will be provided on this webpage once content is confirmed.

Join health care professional, community volunteers, and other interested individuals for a day of collaborative networking, learning, and sharing of Advance Care planning experiences and ideas.

Target Audience
Physicians, nurses, social workers, case managers, health facility administrators, clergy, chaplains, and other interested health professionals.

Program Description
This continuing education program will explore the current state of advance care planning in Indiana including resources available across the state, programs that are available to facilitate advance care planning conversations, discussion of new ideas related to planning, family/caregiver/healthcare representative perspectives related to advance care planning, recent changes to the law in Indiana, and the future of advance care planning in Indiana.
