THE 25th ANNUAL MIDWEST CARE COORDINATION CONFERENCE

Wednesday, May 8, 2019

Kathleen Fraser MSN, MHA, RN-BC, CCM, CRRN, Executive Director of Case Management Society of America (CMSA) and President and CEO of Fraser Imagineers, will provide the keynote address.  The conference will focus on the role of care coordinators, case managers and navigators through all phases of care delivery.  Attendees will be able to implement effective care coordination strategies for a variety of clients.

In addition to a keynote and concurrent presented by Ms. Fraser, a variety of topics will be covered including State Health Insurance Assistance program (SHIP), considerations for the aging population presented by SWIRCA, Ethics, Opioid Crisis and a Homelessness panel discussion.

Jason Koger will close the conference with an inspiring, motivational journey.  A native of Owensboro, Kentucky, Jason will share how a traumatic injury resulted in bilateral arm amputations and his perseverance to live life to the fullest.  Jason became the first bilateral arm amputee to receive app controlled bionic hands.

This conference is appropriate for Case Managers, Care Coordinators, Navigators, Discharge Planners, Nurses, Social Workers, Advanced Practice Nurses, Physicians, Students and any healthcare provider concerned about coordination of care.
