Anticoagulation: Current Practices for Bridging Between Blood Thinners
Bridging therapy in anticoagulation is often needed for two common scenarios. One scenario is with acute therapy for venous thromboembolism (VTE), and the other surrounds perioperative/procedural care. The content of this program examines the purpose of bridging therapy, differentiates between the types of bridge therapy, reviews scoring tools for risk stratification, and outlines common examples of appropriate use of bridge therapy.

Objectives
At the conclusion of this program, participants will be able to:
- Differentiate bridge therapy by describing the difference between acute bridge therapy for treatment of new VTE vs. perioperative bridge therapy
- Explain the primary purpose of perioperative bridge therapy and considerations that can guide clinical decision making
- Identify validated scoring tools that allow for objective risk stratification
- Outline common examples of clinical interventions related to appropriate use of bridge therapy
