Student Athlete Transfer Request

For student athletes transferring out of USI wishing to request their own student records, Title IX or conduct-related records may be requested with the Student Athlete Transfer Request form. 

Please complete the FERPA Request and Release and attach to your Student Athlete Transfer Request.
