Students not meeting course or clinical expectations will be formally notified of their deficiencies and a Performance Improvement Plan will be developed that addresses these deficiencies. The Performance Improvement Plan should include measurable student outcomes, a timeline and expectations to meet the course requirements.

Course and clinical faculty will prepare the Performance Improvement Plan, review it with the student, and secure the student’s signature. Copies of the completed plan will be added to the student’s file. The student will also receive a copy and have the opportunity to include any comments regarding the Performance Improvement Plan. Any email communication with the student should also be recorded in the student’s file.

In a course, a student may receive one Performance Improvement Plan. This Performance Improvement Plan will include a written plan that specifies specific behaviors in which the student must meet in addition to all of the clinical objectives by the end of the course. Failure of the student to meet the objectives outlined in the Performance Improvement Plan or failure to meet any of the clinical objectives of the course will result in a course failure.

If a student receives a third Performance Improvement Plan at any time during the program the student’s individual case will go before the Undergraduate Admission and Progression Committee.
